DENTAL INSURANCE

PLAN OPTION ONE | DELTA DENTAL HIGH PLAN

Calendar Year maximum: $1250; Has lifetime orthodontic benefit of $1000 up to age 19; Coverage is 100% for Diagnostic and Preventative services (see plan summary for details); Coverage is 80% for restorative, oral surgery, periodontics, endodontics, prosthetic maintenance, and emergency dental care (see plan summary for details); Coverage is 50% for Prosthodontics and Major restorative (see plan summary for details),

 

PLAN OPTION TWO | DELTA DENTAL LOW PLAN

Calendar Year maximum: $1000; Coverage is 100% for Diagnostic and Preventative services (see plan summary for details); Coverage is 60% for restorative and oral surgery (see plan summary for details); Coverage is 60% for prosthetics maintenance (see plan summary for details).

 

OUR RECOMMENDATIONS

We recommend contacting Delta Dental directly or your doctor's office to determine if your dental providers are included in your plan. When inquiring about your coverage, it would be helpful to mention the specific plan name, such as "Delta Dental High" or "Delta Dental Low. "


Check on deductibles and coverage that affect your pay when you get services.


Most plans limit benefits expenditures per calendar year (or per lifetime, if orthodontic benefits). Once this calendar-year limit is reached, no additional services will be covered until the following year.

KEEP IN MIND

Once you choose a plan, you cannot change it during the year unless you have a qualifying status change or move out of the plan’s service area, or are retiring and becoming eligible for Medicare. If your doctor leaves your plan, you must find a new participating provider in your chosen plan.

Annual open enrollment for Dental Insurance occurs in April of each year, and the policy becomes effective on July 1.
