CRIMINAL OFFENDER RECORD INFORMATION (CORI)

We are committed to maintaining the safety and security of our community at FRAMINGHAM PUBLIC SCHOOLS. As part of our safety and security efforts, all prospective and current employees, student teachers, contractors and vendors with direct contact with students must undergo a criminal offender record information (CORI) check.

This important check evaluates your criminal history records, and the Department of Criminal Justice Information Services (DCJIS) will review your personal information. By signing this acknowledgment form, you grant Framingham Public Schools the permission to conduct the CORI check on your behalf. This authorization will remain valid for one year from the date of your signature. If you wish to withdraw your consent at any time, please provide written notice to Framingham Public Schools. We want to assure you that any information obtained from this check will be treated with strict confidentiality and used solely for the purpose of safety and security. 

Procedures
Please complete the form below and submit it to either the school's main office or our office. You will receive a notification once it has been processed. If further examination of your CORI is necessary, we will send you a formal letter requesting a written explanation of the circumstances surrounding the offense or data under scrutiny. Please note that until you receive written clearance, you will not be able to participate in any district activity.

Community Volunteers, Parent Volunteers, Student Teachers, Student Interns, Vendors, or Third Parties are required to undergo a background check by filling out one of the forms below:

Keep in mind
A government-issued photo ID must be presented to the CORI recipient and attached to the form.
The CORI is valid for three years.
